Fix This is being caused by codec packs like Combined Community Codec Pack (CCCP) and/or Kawaii Codec Pack (KCP).
If using KCP, go into KCP Settings where you installed the KCP pack. Click LAV Splitter, uncheck AVI.
If using CCCP, search LAV Splitter Settings in your start menu. Then uncheck .mp4 from the filetypes.
